Models That Made It Replication Guidebooks

Replication Guidebooks detailing how to establish and operate each of VHCF's 10 Models That Made It can be downloaded below, or are available by contacting the Foundation.

Prior to applying for funds to replicate one of these models, it is highly recommended that interested applicants review these guidebooks and call the projects directly to learn firsthand about their experiences and to arrange a site visit. In addition, VHCF periodically holds replication workshops, according to interest and need.


As with all proposals submitted to the Foundation, it is important for prospective grantees to demonstrate that essential project elements have been well thought-out and that community support is in place.



Dental Care Models

Accomack’s School-Based Dental Program -
A collaborative venture which provides dental care for public school children in Accomack County.

Carilion Pediatric Dental Program -

A model that provides an innovative approach to dental service delivery via a partnership between a benevolent sponsor and a community coalition.

Free Clinic of the New River Valley’s Adult Dental Program
A dental clinic which utilizes a combination of paid dental staff, local on and off-site volunteer dentists, and dental hygiene students to maximize the number of patients who receive dental care.

Medical Care Models

Free Clinic/Mid-Level Provider Initiative -
Addition of a paid Nurse Practitioner or Physician’s Assistant to augment the services of volunteer health professionals, thereby expanding a free clinic’s patient capacity.

Jefferson Area Board for Aging’s Geriatric Assessment and Intervention Team (GAIT)
Comprehensive case management provided by a multi-disciplinary team of health professionals (a nurse case manager, nurse practitioner, and geriatric social worker) to assess and improve the health and well being of mobile and homebound senior citizens and their caregivers.

Nelson County’s Health Depots -
A circuit riding nurse practitioner who travels from fire halls to senior centers to provide care to those without access.

Olde Towne Medical Center -
A community based health clinic which uses paid and volunteer health professionals to provide health care and support services for uninsured and underserved patients.

Mental Health Care Models

Mental Health Association of the New River Valley Pro Bono Mental Health Program – Mental health services for low-income uninsured patients provided by on and off-site volunteer professionals and license-eligible mental health trainees.

*Ancillary Service Models

Med-Ride - A non-emergency medical transportation network which uses volunteers and local agencies to serve five rural communities in the New River Valley, covering 1400 square miles.

Cross-Over Health Center’s Lay Health Promoter (LHP) Initiative - A 10-week course which trains and educates inner-city residents to identify health problems and help their neighbors access care.

*While these models are outstanding, VHCF is currently targeting its funds to projects which provide direct services rather than the ancillary services provided by these models.
