Models That Made It

While many VHCF grants fund innovative approaches to local health needs, the Virginia Health Care Foundation also provides grants to replicate uniquely successful initiatives.

VHCF has identified 10 Models that Made It -- projects which, in addition to being cost-effective, successful at meeting primary care needs, and capable of sustaining themselves, also can be replicated in communities across the state.  

Organizations are encouraged to apply to VHCF for grant funding to replicate a Models that Made It project targeted to addressing a community's needs in these areas:

Dental Care
Medical Care
Mental Health Care
Ancillary Services*

Interested in applying for VHCF funding to replicate one of these Models?  Please review the VHCF Grant Guidelines and keep in mind that, as with all proposals submitted to the Foundation, prospective Models That Made It grantees must demonstrate that essential project elements have been well thought-out and community support is in place. 

*While the Ancillary Service Models are outstanding, VHCF currently is targeting funding to projects that provide direct services.
