The Virginia Health Care Foundation is always ready to share great ideas. Ten projects funded by the Foundation have proven to be cost effective, successful at meeting primary care needs, capable of sustaining themselves and replicable. These projects are the Foundation’s Models That Made It.

VHCF encourages interested communities to consider replicating these Models That Made It, which fall into four categories:

• Dental Care
• Medical Care
• Mental Health Care
• Ancillary Service*

Interested in applying for VHCF funding to replicate one of these Models?  Please review our Grant Guidelines and keep in mind that, as with all proposals submitted to the Foundation, prospective Models That Made It grantees must demonstrate that essential project elements have been well thought-out and community support is in place.
