Grant Reports, Forms & Deadlines
VHCF typically funds projects for three years. During each funding year, grantees are required to submit two progress reports. Grantees also have an opportunity each year to submit written requests to receive continued funding (years 1 and 2), use unspent funding, or, in rare instances, expand the project with additional funding.
6 Month and Yearend Progress Reports
With each progress report, VHCF seeks to evaluate how the project is doing in general; its successes and challenges; how well it is able to complete specific project objectives and goals as spelled out in
the Letter of Agreement; and how the project funds have been spent.

6 Month Progress Report: This comprehensive report covers grant activities for the first 6 months of the grant year. |

Yearend Progress Report: Structured in the same format as the First Progress Report, this report covers the full 12 month grant year. |
Continuation Requests
The Continuation Request is used (in addition to community partner surveys and the site visit) in the decision to award a continuation (if applicable). Continuation of funding is not automatic and must be applied for each year. Determinations of continued funding are based on a project’s performance to-date.
Extension and Expansion Requests
Grantees seeking to utilize unexpended VHCF grant funds, or to receive additional funding to expand a project beyond its original scope must submit an Extension or Expansion Request.

Extension Request: Extensions authorize a grantee to utilize unexpended VHCF funds. These requests must be filed by grantees that will not spend their entire awards by the end of the grant cycle. If awarded, these funds are made available in a fourth grant year. Extension Requests are due with the Continuation Request. |

Expansion Request: Expansion Requests must be discussed with a grantee's program officer prior to submission. Expansions may be granted to expand the existing project in the event that the demand far exceeds the initial goals or projections. If approved, Extension Requests are due with the Continuation Request. |
