Greater Williamsburg MAP - Olde Towne Medical Center
The Greater Williamsburg Medication Assistance Program (GWMAP) is a partnership between the Virginia Health Care Foundation and the Williamsburg Community Health Foundation.
Olde Towne Medical Center, as part of the GWMAP, assists eligible individuals from the Williamsburg, James City and York County areas with accessing free medications from pharmaceutical companies’ patient assistance programs via VHCF's The Pharmacy Connection.

Halifax Regional Development Foundation
Halifax Regional Development Foundation will use RxRelief Virginia funding to establish a Medication Assistance Program by employing a Medication Assistance Caseworker (MAC). The MAC will assist eligible individuals, primarily from Halifax County, with accessing free medications from pharmaceutical companies’ patient assistance programs by using The Pharmacy Connection.

Hampton Roads Ecumencial Lodgings and Provisions, Inc.
The Hampton Ecumenical Lodgings and Provisions (HELP) Free Clinic and the Peninsula Christian Free Clinic received a RxRelief Virginia grant from VHCF to establish a Medication Assistance Program. This funding enabled them to go from a .5 FTE Medication Assistance Caseworker (MAC) to a full-time MAC shared between the two clinics providing both evening and daytime hours. The MACs utilize the Pharmacy Connection software supplied by VHCF to access free medications from pharmaceutical companies’ patient assistance programs. Now the clinics are able to meet the long-term prescription medication needs of their uninsured, chronically ill patients from Hampton and Newport News.

Harrison-Rockingham Dental Clinic
Harrisonburg-Rockingham Dental Clinic received grant funding from VHCF to support the salary of a second part-time dentist. This addition has enabled the clinic to offer dental care five days a week to children with FAMIS insurance as well as providing care to uninsured adults.

Harrisonburg Community Health Center
In 2007, a coalition led by the Institute for Innovation in Health and Human Services at James Madison University (JMU) with support from Rockingham Memorial Hospital (RMH), created The Harrisonburg Community Health Center to serve patients in Harrisonburg and Rockingham County. This area has been particularly impacted by the limited number of pediatricians accepting new patients with FAMIS/FAMIS Plus.
The center opened in January 2007 with a part-time pediatrician, funded by VHCF, to serve the medical needs of area children; and a full-time nurse practitioner to meet the adult medical care needs. VHCF has now awarded funding to help underwrite the salary and benefits of a full-time pediatrician to provide care to the many children for whom it serves as a medical home.

Harrisonburg-Rockingham Free Clinic
One of the first grants issued by VHCF helped establish the Harrisonburg-Rockingham Free Clinic. Several years later, a second grant enabled this free clinic to add a half-time nurse practitioner to expand clinic services.

Health Care on the Square - Boydton Medical Center
VHCF funding enabled the Boydton Medical Center to hire a full-time dentist and a part-time dental hygienist to staff this community health center's new dental facility, Health Care on the Square.

Henrico Area Mental Health & Retardation Services
VHCF is provided funding through its mental health initiative, A New Lease on Life, increase access to basic mental health services and prescription medications for Cross Over Free Medical Clinic patients, focusing on providing services to existing clinic patients with co-occurring diabetes and mental illness.

Henrico County Public Schools
A Project Connect grant is enabling Henrico County Public Schools to reach and enroll school-aged children in FAMIS and FAMIS Plus child health insurance through school-based outreach strategies. The district will also work on institutionalizing outreach and enrollment for FAMIS/FAMIS Plus into the work of school nurses and clinic aides.
