Taking Aim, Improving Health:

The Virginia Health Care Foundation (VHCF) awarded six grants under its population health initiative, Taking Aim, Improving Health (TAIH). Its purpose was to stimulate and/or strengthen collaboration between hospitals and their local primary care health safety net organizations (free clinics and community health centers) to reduce avoidable hospital admissions and Emergency Department visits by uninsured patients via population health initiatives.

The six TAIH grantees used differing approaches to intervene with a total of 1,095 high-cost charity care patients during the 18-month grant period. TAIH was designed and implemented with the input, advice, and full engagement of leaders from the Virginia Hospital & Healthcare Association, the Virginia Association of Free and Charitable Clinics, and the Virginia Community Healthcare Association.

• Eastern Shore Rural Health System and Riverside Shore Memorial Hospital
• Sinclair Health Clinic (formerly Free Medical Clinic of the Northern Shenandoah Valley) and Winchester Medical Center
• Lloyd F. Moss Free Clinic, Mary Washington Hospital, Stafford Hospital, and Spotsylvania Regional Medical Center
• Martinsville Henry County Coalition for Health and Wellness and Memorial Hospital of Martinsville and Henry County
• New Horizons Healthcare, Bradley Free Clinic, Carilion Clinic, and United Way of Roanoke Valley (Healthy Roanoke Valley)
• Norton Community Hospital, Lonesome Pine Hospital, The Health Wagon, and Frontier Health
